News just out suggests that a Jack Russell cross was laying in front of the fan to cool off (after chasing the cat) and got his tail caught in the blades. If your Jack Russell lives through out your house, consider putting fans on pedestals so they are up and out of the way of passing paws, tails, fingers, toes, etc.
